What you need: (to enroll your spouse and/or children)

A certified copy of a Birth Certificate for your children

A certified copy of a Marriage Certificate for your spouse

What are some of the main differences between the 2 plan options?

CO-PREMIUM: The monthly co-premium is different for each Plan. Your monthly co-premium is the amount of money taken out of your paycheck each month. The Hospitality Gold Plus Plan has a lower monthly co-premium than the Kaiser Southern California Plan.

Tier Hospitality Gold Plus Plan Co-Premium Kaiser Southern California Plan Co-Premium
Single: $111.06 $129.24
Single + Spouse: $236.87 $275.62
Single + Child(ren): $185.26 $215.57
Family: $328.99 $382.81

NETWORK: The network of doctors and providers is different for each Plan. With the Hospitality Gold Plus Plan, you would have access to the national Blue Cross Blue Shield network (Blue Cross providers in California). With the Kaiser Southern California Plan, you would have access to the Kaiser network. If you choose the Hospitality Gold Plus Plan, you can choose to go to an in-network or out-of-network doctor. However, staying in-network would save you money. If you choose the Kaiser Southern California Plan you can only go to a Kaiser doctor or pharmacy. Both plans have great networks! It ultimately just depends on what you prefer.

COPAYS: The copays are different for each Plan. Your copay is what you pay when you go to the doctor or get care. You usually pay it during your visit. Some of the copays for each Plan are listed below:

Service Hospitality Gold Plus Plan Copay/‍Charge (In-Network) Kaiser Southern California Plan Copay/‍Charge
Preventive Care: $0 $0
Primary Care Visits: $20 per visit $20 per visit
Specialist Visits: $40 per visit $20 per visit
Urgent Care Visits: $40 per visit $20 per visit
Emergency Room Visits: $150 per visit (waived if admitted) $150 per visit
Hospital Stays: $250 per day ($750 max per admission) $500 per admission
